Participation in private lessons is contingent on completing this online purchase form plus a signed release & waiver and health form. (Provided separately)

Refund policy: If COVID causes the cancellation of lessons, a full refund will be granted. Cancellation by participant for any reason will result in a credit for a comparably priced future Lakeside lesson.
